Tumescence TRAP is one of the complementary procedures of Three-dimensional Regenerative Ambulatory Phlebotherapy (TRAP).
As is known, TRAP has revolutionized phlebological treatment by replacing irrational techniques, such as sclerotherapy, phlebectomy, saphenectomy, so-called functional ligature, burning by means of laser and HF currents, and the recent absurd procedures of gluing the veins.
TRAP is a biological technique that treats the venous walls, making the veins continent once again. Its action is three-dimensional, extending to the perforating, communicating and superficial circulation. TRAP treats the hemodynamic hypertension of the venous circulation in the lower limbs.

The elastic thread does not cut into the tissues and is transformed into a ligament that stabilizes the result. With the two-tipped Jano needle, there is no need to dissect the tissues, and we can act close to the defect that we want to correct. Elastic liftings are effective, minimally invasive, ambulatory and have long-lasting results.

Correction of drooping cheeks and round eyes. An elastic MACS lift and elastic canthopexy are carried out. The point of anchorage of the elastic thread is marked very close to the external canthus or immediately below it. The apex of the malar suspension triangle and the two apexes of the triangles that will lift the cheeks are marked out. The bisectors of the Isosceles triangles are also drawn.
The elastic threads (Elasticum EP3,5 – EP4, Korpo) are implanted. The first Isosceles suspension triangle is the one used for canthopexy. After elastic MACS lifting, no bandaging is required; only an ointment is applied to the incisions. As the threads do not cut into the tissues, and are transformed into ligaments, the result is permanent.
If necessary, elastic MACS lifting can also be performed in the third decade of life, as it is minimally invasive and prevents gravitational ptosis.

This patient wants to elongate her eyes and correct the exposure of the sclera. With aging, the eyes lose their almond shape and become round. Moreover, if adipose pouches are removed from the lower eyelids by means of skin incisions, this frequently results in loss of the elongated shape of the eyes. To correct this undesirable appearance, we have designed elastic canthopexy, a less invasive technique than traditional canthopexy procedures, and one which is able to rejuvenate the eyes and, as in this case, modify their slope.
In this procedure, traction is exerted not only on the external canthus, as in normal canthopexy procedures, but also on the tissues lateral to the canthus that come within the Isosceles traction triangle. When the eyes are lengthened and raised, the skin of the lateral portion of the upper eyelids often presents some drooping. For this reason, a few months later, when stabilisation is complete, we perform upper blepharoplasty, a technique published in the section “Timedsurgery”.

This patient underwent elastic canthopexy to elongate and raise the external angle of her eyes, correct their round appearance and reduce exposure of the sclera. Raising the eyes increases the lateral amount of drooping skin of the upper eyelids. This defect is eliminated by means of exclusively cutaneous upper blepharoplasty.
The Timed apparatus (Korpo) is set to the function of rapid pulsed cutting. A 0.08 mm diameter EM10 White electromaniple is used and the power is set to 27 Watts. This special mode of pulsed timedsurgical cutting does not burn the edges of the cut, enables millimetric precision to be achieved and exerts negligible pressure on the tissues. After 3-6 days, the stitches are removed. The patient now has a more charming look. Her beautiful green eyes prompt us to suggest further rejuvenation of the region through the elimination of her lower eyelid wrinkles. For this purpose, the technique of choice is timedsurgical mixed peeling with de-epithelialization, illustrated in the Timedsurgery section of the Medical Video Journal.

The preoperative design shows: the incision that crosses the sideburn and stops at the tragus, the pathway of the elastic canthopexy thread, the parallel neck threads, and the vectors of the isosceles triangles that suspend the cheeks and the malar region.
The skin incision that crosses the sideburn and stops at the tragus is now made. This incision is beveled to maintain the integrity of the hair follicles of the sideburn.
The subcutaneous tissue is dissected from the superficial temporal fascia with scissors. The dissected area is rectangular in shape and extends slightly beyond the zygomatic arch; it is not extended medially, to avoid interfering with the functioning of the frontal branch of the facial nerve. Dissection is normally carried out with large scissors, as my mother, Dr Spolidoro, used to teach.
The operator uses a finger to check whether the dissection has extended beyond the zygomatic arch.

Timedsurgical mixed peeling is the technique of choice for the permanent removal of deep wrinkles of the lip.
Just one treatment is required for excellent results that last years. After the mixed peeling procedure, the patient simply needs to dry off the area with a tissue every few hours; no other medication or substances are needed.
In rare cases, as in this patient, a thin wrinkle may remain. This can be re-treated with timedsurgical mixed peeling. The results of this standardized timedsurgical procedure by the Timed Korpo apparatus cannot be obtained by using any other technique. The wrinkles disappear, the skin is rejuvenated, and the results are permanent.
